FAQs about Grace Center for Adult Day Services
- How can I get more information about Grace Center?
- It’s easy--just call our office at (541) 754-8417 and a staff member will be happy to answer your questions! The best time to call is between 8:00 a.m. and 4:00 p.m. Monday through Friday when our Executive Director/R.N. or our Activity Director are usually available to take inquiries. We have written information and a video (DVD or VHS) that we can mail to you as well. If you would like to see our center, please call ahead so that we can be sure that someone is available to give you a tour and answer your questions. We can schedule a Complementary Visit for you to try our services for a day!
- My husband is only 50 years old and recently finished therapy following a severe stroke. Is this just for seniors, or do you have younger adults in your program?
- Yes, we currently have many participants between the ages of 35 and 65 who have had strokes or other types of brain injuries. Our Post-Rehabilitation Program picks up where therapies leave off, to help participants continue to maintain or improve upon their physical and mental abilities.
- My mother has pretty severe memory loss and can’t do much of anything for herself. Is she too far advanced for your program?
- Probably not, but we will need to do an assessment of her needs. Our Memory Care Program is designed to provide physical and mental stimulation for people with moderate to severe memory loss. We have staff who are very experienced and compassionate in providing care for people with Alzheimer’s disease and other dementias.
- My wife uses a wheelchair and cannot walk or get herself on or off the toilet. Can she still come to your program?
- Yes, we have a mechanical sit-to-stand lift and qualified staff who can transfer her to the toilet and to a recliner if she needs to rest during the day. We also have a device called a gait-harness ambulator that can sometimes help non-ambulatory participants to walk without danger of falling.
- What are your hours of operation and where are you located?
- We are open from 8:00 a.m. to 4:30 p.m. Monday through Friday and are closed only on major holidays. For an additional fee, services can be provided until 5:00 p.m. by special arrangement. We are located at 980 N.W. Spruce Avenue, behind the Richey’s Market & Rite-Aid shopping center. Spruce Avenue runs parallel to Circle Boulevard. between N.W. 9 th Street. and Highland Avenue.
- Do you have attendance or scheduling requirements?
- Yes, in order to provide staffing, food, and materials needed for high-quality care, we need for you to set up a schedule. You can choose how many days per week and which days you would like to come. You can change your schedule if something comes up and you need different or additional days.
- What does it cost to attend the program?
- We offer a Complimentary Visit, so there is no charge for your first day! Our daily fee is $76, and we offer discounts of $10, $21 and $31 per day based on income. Long term health insurance policies usually pay for adult day services, and Veterans Services may be able to help. We also have a contract with the State for Medicaid clients, and Oregon Project Independence may also be available. We offer half-days from 8:00-11:30 a.m. for $40 with optional lunch from 11:30-12:30 for $10 or 1:00-4:30 p.m. for $40 with optional lunch from 11:45- 1:00 p.m. for $10.
